INFORMATION REQUIRED FOR US DRIVERS FILEYour driver's files must contain the following documents: Application Form All driver's files must contain a completed employment application form. Failure to have this document will result in a fine and points infraction on the carriers’ C.V.O.R. in the event of a M.T.O. audit. Drivers Abstract All driver’s files are required to contain a valid abstract (updated yearly) in the qualification file. Failure to have this document in each file violates M.T.O. and US D.O.T. F.M.C.S.R. carrier requirements under the driver qualification section. Licence Photo All driver’s files are required to contain a photocopy of the drivers licence. This is a requirement of the National Safety Code for C.V.O.R. carriers. Certificate of Review Your driver’s files were missing the Annual Review Of Driving Record as required by all I.C.C. authorized carrier in accordance with the US F.M.C.S.R. Section 391.25. This annual review must be filled out and completed on a yearly basis. D.O.T. Road Test All driver’s are to be given a road test to demonstrate their safe and proper handling of equipment. A copy of the road test must be retained on file. This is a requirement of ORIC and the US D.O.T. for I.C.C. licensed carriers. Drivers Data Sheet All driver’s or owner/operators who are recently hired must have a drivers data sheet completed and added to the driver qualification file. This is a requirement under the F.M.C.S.R. in section 395.8 for all I.C.C. Request from Previous Employer All I.C.C. driver qualification files must contain a request for information from previous employer sheet filled out and added to the driver file. This is a requirement found in the F.M.C.S.R. under section 391.23. Failure to have this document in the drivers file could result in a fine by the US D.O.T. when conducting an audit. F.M.C.S.R. It is recommended that all driver’s and owner/operators carry a copy of the F.M.C.S.R. in the truck when operating in the US. Also have the driver sign for this booklet indicating that he will keep it in his possession at all times while operating a tractor trailer on the carriers behave.
